- Download request form (complete, scan and return to Eastrad).
- Phone or email the practice to determine and confirm an appointment date
- MRI requests can be emailed to mrireception@tci.co.za
- CT scan & Other requests can be emailed to reception@eastrad.co.za
- Please note, MRI and CT scans require a referring specialist
- Request form booklets for clinicians and access to imaging available on request
